Default MR Loan agreement enforceability

Is the following either executable or compliant with the act.

The original is not signed by the Bank, it doesn't contain 'the full cost of credit' and the original sent in response to my CCA request did not have our signatures and this recent one 'magically' does, all looks really 'moody' to me.
